A health certificate is not entered in the passport-number field and the current service form has no vaccination upload. Arrange the appropriate travel health documentation through the relevant official service, independently of the eVisa request.
Brazilian passport names, not CPF details
Use the passport’s separation of sobrenomes and given names. Include the printed family-name components in the same order, even when a shorter version appears in your airline account.
The form asks for a passport number, not a CPF or RG number. Check the letters and digits of that passport, then enter a date of birth that belongs to the applicant. When helping relatives, change the personal details for every traveller rather than reusing a completed record.

Ask your insurer to confirm coverage in Cuba and explain how to obtain assistance there. Keep the policy information with the health certificate, return itinerary and completed arrival declaration.

Yellow fever evidence belongs in the travel file, not a visa upload
For a journey departing Brazil, review the Brazilian embassy's current health-document guidance early enough to resolve certificate questions before the flight. Confirm the requirement, applicable exceptions and acceptable evidence with the relevant authority and carrier. A vaccination appointment record and the international certificate required for travel are not necessarily the same document.
The current service application has no vaccination-certificate upload. Keep any required health evidence separately with the passport and airline documents. Completing the passport-detail form therefore cannot be used as confirmation that the airline has reviewed a traveller's vaccination or health exemption evidence.
Example: a Brazilian resident uses a different nationality passport
A traveller living in Brazil but carrying a Portuguese passport should not select Brazil merely because the departure flight starts there. Use the passport nationality for Cuban eVisa eligibility, while checking health and transit rules that may depend on the itinerary. These are two distinct questions: which passport is applying, and where the passenger has travelled or will depart.
For a Brazilian passport application, use its document number and full name fields rather than a CPF or identity-card entry. If several people travel together, compare each person's passport separately. A group payment or shared contact email does not make the travellers' identity details interchangeable.
